Please note that the blank Certificates of Origin forms are sold by the Chamber in a pad of 100, and the blank Copy Certificates of Origin forms are sold in a pad of 200.

Please also note that both types of certificates are subject to VAT (17.5%) and a charge of £5.00 P & P per pad if your forms are sent to you by post.

One pad of blank Original Certificate of Origin forms = £70.00 (70p x 100)

One pad of blank Copy Certificate of Origin forms = £80.00 (40p x 200)
